Male infertility testing and treatments at KIMS

Male Fertility Evaluation

Male infertility can result from various factors like poor sperm quality, blockage, hormonal issues, or testicular failure respectively. Traditional semen analysis may not always show the full picture, which is why we offer more sophisticated diagnostic tools like DFI and MACS. Treatments like TESA and Micro TESE are minor surgical procedures that allow sperm retrieval in cases where it's not present in the semen. Together, these tools give couples a stronger chance at successful conception, even in complex cases.

Benefits

  • Precise Diagnosis: Identifies causes like low motility, poor morphology or DNA damage.
  • Targeted Treatment: Helps plan ICSI, IMSI or surgical sperm retrieval where needed.
  • Sperm Health Insight: Detects hidden sperm defects not found in basic semen analysis.
  • Improved Fertility Outcomes: Increases fertilisation and embryo quality when combined with IVF.

How to prepare for the diagnosis?

  • Semen Analysis: First step to assess count, motility and structure.
  • Hormonal Profile: Blood tests help check testosterone and related hormone levels.
  • Scrotal Ultrasound: May be done to assess physical blockages or varicocele.
  • Avoid Heat & Toxins: Limit exposure to saunas, laptops on laps, and chemicals.
  • Nutrition & Supplements: Zinc, folate and antioxidants can support sperm production.

The Procedures

  • DFI (DNA Fragmentation Index) evaluates the integrity of sperm DNA to detect damage not visible in standard tests.
  • MACS (Magnetic-Activated Cell Sorting) separates healthy sperm from those undergoing programmed cell death, improving fertilisation success in IUI or IVF.
  • `TESA` is a minimally invasive procedure that retrieves sperm directly from the testicles using a fine needle.
  • Micro TESE (Microsurgical Testicular Sperm Extraction) is a more advanced surgical method done under a microscope, used when TESA is unsuccessful or in non-obstructive azoospermia cases.

Each of these options is selected based on individual evaluation, and guided by our experienced team of fertility doctors.

General Success Rate

  • ICSI using surgically retrieved sperm (TESA or Micro-TESE): 30-50% per IVF cycle
  • IMSI and PICSI methods may improve embryo quality, implantation and clinical pregnancy rates by 10-15% in select cases

Success varies with the degree of sperm abnormality and female partner's fertility health.

Frequently Asked Questions

What is DNA Fragmentation Index (DFI) and why is it important?
×
DFI measures the level of DNA damage in sperm. High fragmentation can reduce the chances of fertilisation, implantation, or lead to miscarriage. It's especially useful if IVF or ICSI has failed previously.
Is MACS useful for natural pregnancy or only IVF?
+
MACS improves sperm selection in assisted reproduction like IUI or IVF. It’s not applicable for natural conception but greatly helps couples facing repeated failures or poor embryo quality.
What’s the difference between TESA and Micro TESE?
+
TESA uses a needle to extract sperm from the testicles. Micro TESE is more detailed, performed using a microscope to find sperm in men with very low or no sperm count. It’s used when TESA fails or in more severe cases.
Will these procedures affect my testosterone or sexual function?
+
No. Both TESA and Micro TESE are safe when performed by experienced specialists. They do not interfere with hormonal levels or sexual performance.
How do I know which procedure is right for me?
+
Your doctor will recommend tests like semen analysis, hormone levels, and ultrasound. Based on your condition, we may suggest DFI, MACS, TESA, or Micro TESE. The approach is always individualised.
Can male infertility be treated with medication alone?
+
In some cases, yes. If the cause is hormonal imbalance or infection, medications can help restore sperm production or improve quality. However, for more complex issues like azoospermia or severe DNA fragmentation, procedures like TESA or advanced sperm selection may be needed.
Is it necessary to repeat these tests if we’ve already done a semen analysis elsewhere?
+
It depends on the detail and timing of the previous test. A basic semen analysis may miss deeper issues like DNA fragmentation. If your fertility journey has faced delays or failed treatments, repeating tests at a specialised centre can offer more clarity.
How long should I wait after lifestyle changes before rechecking sperm quality?
+
Sperm regeneration takes about 72–90 days. If you’ve made changes like quitting smoking, improving your diet or reducing stress, it’s best to retest after 3 months to accurately assess improvements in sperm quality.
