Ovulation Induction with Timed Intercourse (OITI) is an assistive method for natural conception, where the whole cycle of the woman is mapped accurately with ultrasound imaging over the course of a month. Then, we advise the couple to have unprotected intercourse before and during the days of ovulation. If you have unexplained infertility issues, then this can be a great first step to try, without the need for any complicated treatments initially.

How to Prepare for OITI

  • Baseline Scans: An ultrasound is done on Day 2 or 3 of your period to assess ovarian health.
  • Fertility Tests: You and your partner may undergo basic fertility testing including hormone levels and semen analysis.
  • Healthy Lifestyle: Maintain a nutritious diet, reduce caffeine and alcohol, and quit smoking.
  • Track Menstrual Cycle: Keep a record of your periods to help estimate ovulation timing.
  • Stay Relaxed: Stress can impact ovulation. Practice relaxation techniques like meditation or light exercise.

OITI Procedure

Our fertility specialist will begin treatment on the 2nd day of your menstrual cycle. You will then need to take induction medications/injections for 1-2 weeks. Regular ultrasounds are taken at this time to track your cycle accurately. You will then be asked to have intercourse on specific days - just before and during the ovulation period, so there is a higher chance of the sperm fertilising the released egg and forming an embryo. Don't get too stressed if it does not happen in one round. Stay positive, so there is less stress and try again next month! A general rule of thumb is to wait for 2-5 months, before trying other treatments.

  • Success per cycle: 10-20%, depending on age, egg and sperm quality, and how accurately ovulation is tracked.
  • Most couples are advised to try OITI for 3-6 cycles before considering other fertility treatments.
  • Best results are seen in women under 35 with regular cycles and no major underlying fertility issues.

What is OITI?

Ovary-induction with timed intercourse is a strategy where you are asked to time your intercourse and also given ovary stimulants- so that you release one or more eggs. With this higher number and the proper timing, there is a greater chance that you will get a fertilized embryo.
When is OITI recommended for couples?

If you have unexplained infertility, PCOS, cervical factor infertility or irregular cycles, then OITI is recommended.
How do I know if OITI is right for me?

If you do not want to try IVF or other advanced modes of assisted reproduction, and have good ovarian reserves, but have issues with unexplained infertility, then OITI is a good choice as there is very minimal intervention used in this method.
Will OITI work if I’m trying to get pregnant for the first time?

Generally, couples tend to get pregnant within a year or so of having unprotected sex, if all other parameters are fine. You just need to know when your fertile window is (it lasts for about a couple days or slightly longer every month), which makes timing the all-deciding factor. So, yes, you can get pregnant with OI-TI (if all other conditions are met) but it may not happen right away. You may have to wait for a few months to get some good news. Talk to us if you think another approach may be better for you.
What are the risks associated with getting OITI done?

Since more eggs are released than the usual number, multiple pregnancies are a possibility. You may also have OHSS (Ovarian HyperStimulation Syndrome), leading to unwanted side effects like bloating, fluid build up etc. but these can be managed by experienced fertility specialists- either by lowering the dosage of ovulation induction drugs or by giving the hCG later. Some women may also have mood swings and anxiety.
How many OITI cycles should we try before exploring other treatments?

You can try OITI for about 3 to 6 cycles. If you don’t conceive within that time, your doctor may recommend moving to treatments like IUI or IVF for better chances.
Will I need to take medication every time I do OITI?

Not always. Some women may undergo OITI in a natural cycle without medication. However, for others, ovulation induction using tablets or injections may improve the outcome.
Can stress affect the results of OITI?

Yes. High stress levels can interfere with ovulation and hormone balance. Staying calm and maintaining a positive mindset plays an important role in improving your chances of conception.
